What’s New?
- Update 6/19/25 Version 1.0.2 – Fixed a minor issue to show recommendation when to re-dose the next regimen if AUC is > 600
- Update 6/13/25 Version 1.0.1 – Added recommendation when to re-dose for the next regimen when AUC24 is > 600
- Would recommend to re-dose when estimated vancomycin level is approaching approximately 15 mg/L
How to Use
- Input all the data required
- Click Calculate to generate parameters and kinetic model
- Recommended dose/frequency is based on AUC 400–600 mg·hr/L
- Enter your desired dose and frequency, select the preferred ke model, then click Generate to finalize the patient plan Note: For Actual ke calculations, the volume of distribution (Vd) is retained while the elimination rate constant (ke) is adjusted to match the specific time the trough was drawn. This allows the model to more accurately reflect the patient’s observed level, resulting in a more individualized pharmacokinetic profile.
Formulas Used
Vd = (0.29 × Age) + (0.33 × Weight) + 11ke (Single-Point) = -ln((Ctrough × Vd / Dose) ÷ [1 + (Ctrough × Vd / Dose)]) ÷ Tke = ((0.705 × CrCl + 4) × 0.06) / VdHalf-life = 0.693 / keCmax = [Dose / (t' × ke × Vd)] × [(1 - e−ke × t') ÷ (1 - e−ke × τ)]Cmin = Cmax × e−ke × (τ − t')AUC24 = (Dose / (Vd × ke)) × (24 / τ)Alternative AUC24 Checks = [(Cmax + Ctrough)/2 × (Dose/1000)] + [(Cmax − Ctrough) / ke]Custom Trough Goal = (400–600 × Trough) ÷ AUC24
